This Good Faith Agreement is an alliance whereby both parties operate on the premise that there is benefit in all ways possible to both parties. We look and act together to increase the ‘greater good’ of both (the Practitioner) and Mynd Myself.

The intention of this Agreement is to ensure that both parties are clear on what they will contribute to this affiliation and what they can expect from it.

1. Period of Service

(a) The period of service under this Agreement will be from:


(b) If it is mutually agreed to terminate this Agreement, either party may do so by giving the other party thirty (30) days written notice of intention of such action. All monies due to Mynd Myself under this Agreement will be paid on or before the date of actual termination, including all fees and expenses (up to and including the current month of the Agreement). Annual Memberships are non-refundable.

2. Character and Extent of Services

It is understood that:

(a) Mynd Myself will provide an ever-evolving platform that houses credible information about Complementary and Integrative Health and Practitioner Profiles whereby people can connect directly with the Practitioner to help those patients/clients plan and navigate their health journeys.

(b) Mynd Myself’s marketing includes promoting the website via newsletters, multiple Social Media posts each week, regular podcast interviews, blogs, email campaigns, and Myndful Health Panel Discussions.

Mynd Myself also implements SEO strategies to drive organic traffic to the site, Social Media Channel engagement campaigns on LinkedIn, Facebook, and Instagram, along with well-placed and effectiveness-monitored ads on appropriate platforms.

There is a concerted ongoing effort to optimize word-of-mouth-marketing in an informed, planned, and strategic way to increase each Practitioner’s visibility and credibility.


(c) All Practitioner information that is not agreed-to-be-shared-publicly on the Mynd Myself platform will be considered confidential and private. No confidential information will be conveyed to any source without written consent from the Practitioner.

3. Compensation

Mynd Myself provides the Practitioner with the following Membership Paths:

  • One Year Membership (12 months) for an introductory amount of $440 US (plus applicable taxes) OR
  • A Monthly Membership for $40 US (plus applicable taxes) OR
  • An agreed-to Barter Exchange

Initial Practitioner Interview and Podcast Recording Fee

There is an optional, one-time Practitioner onboarding fee of $200 US (plus applicable taxes) for those who wish to have a Profile interview as well as 1 or 2 podcast interviews recorded, edited, and produced with Mynd Myself Founder, Esther Sarlo.

The Profile interview will be positioned on the Practitioner’s Mynd Myself Profile page so site visitors can “meet” them before booking an appointment.

The podcast interviews will be available on the Mynd Myself Podcast and on the Apple and Spotify platforms.

Renewal for Annual and Monthly Memberships

30 days prior to the one-year anniversary date, the Practitioner will receive a notice from Mynd Myself that the Membership will renew soon.

The Membership will automatically renew unless the Practitioner cancels their membership in writing with an email to esther@myndmyself.com. Rate changes may occur and will be indicated in this notification.

Renewal for Barter Exchange

30 days prior to the one-year anniversary date, the Practitioner will receive a notice from Mynd Myself that the Membership is reaching its end. If the Practitioner wishes to renew their Membership they will reach out to Esther to renegotiate terms. Otherwise, the Membership will be ended.

Referral Fees

Effective as of the first day of this Agreement, the Practitioner agrees to pay Mynd Myself the following referral fees:

10% of any fees charged to patients/clients or fees received for consulting or other gigs that have come to the Practitioner as a result of the Mynd Myself platform or any of Mynd Myself’s or Esther Sarlo’s marketing or networking activities.

Referral fees are due and payable on the first day of each month of the Agreement for the duration of this agreement.

All payments are processed through Stripe, or, in Canada, via an etransfer to esther@myndmyself.com.

Any changes to this Agreement after the date of acceptance by both parties will be made in writing and be attached to this document.
